Description
Arising from the postolivary sulcus along the superior-lateral medulla, the glossopharyngeal nerve (CN IX) courses anterolaterally toward the jugular foramen, where it travels in close company with the vagus nerve (CN X) and internal jugular vein. Distal to the skull base it relates to the internal carotid artery and then sweeps forward to the lateral pharyngeal wall, passing between the stylopharyngeus and the superior pharyngeal constrictor on its way toward the posterior third of the tongue. Small branches are logically included, including the tympanic nerve (Jacobson nerve) heading back toward the middle ear and contributions that participate in the carotid sinus region. Short and crowded. This arrangement matters when you need to explain why lesions near the jugular foramen rarely isolate a single cranial nerve, with CN IX palsy often accompanying vagal dysfunction and lower cranial nerve findings in jugular foramen schwannoma, glomus jugulare tumors, or skull base fractures. The tympanic branch and its route via the tympanic plexus to the lesser petrosal nerve anchor a clean teaching line to referred otalgia and to parasympathetic supply of the parotid gland, which becomes relevant in parotid surgery and in neuralgic pain syndromes. Clinically, loss of the afferent limb of the gag reflex and impaired taste from the posterior one third of the tongue remain the bedside correlates. Use this plate when you need a cranial nerves module that does not separate neuroanatomy from vascular relationships, including head and neck anatomy courses, speech and swallowing education, or operative atlases discussing skull base and tonsillar fossa approaches. It also reads well in clinical teaching on glossopharyngeal neuralgia, carotid endarterectomy risk discussion, and differential diagnosis of dysphagia with otalgia.
