Seen from a superior vantage over the skull base, the vagus nerve (cranial nerve X, pneumogastric nerve) is traced as paired trunks descending from the jugular foramen, positioned posterolateral to the glossopharyngeal nerve (CN IX) and anteromedial to the accessory nerve (CN XI) as these structures converge at the pars nervosa and pars vascularis region. The nerve courses inferiorly along the carotid sheath territory, where it lies between the internal carotid artery or common carotid artery medially and the internal jugular vein laterally. Surrounding bony landmarks of the cranial base are included, with the temporal bone and sphenoid forming the foraminal framework that orients the viewer to neurovascular exit points. Orientation at the jugular foramen matters because lesions here rarely respect a single nerve, and this relationship-heavy view helps explain combined lower cranial neuropathies, including Vernet syndrome with dysphonia and dysphagia when CN IX and X are involved. Surgical corridors around the jugular bulb and skull base paragangliomas demand an accurate mental map of how CN X relates to the jugular vein, carotid artery, and neighboring CN IX and XI, since traction or devascularization can translate into postoperative vocal fold paresis via recurrent laryngeal dysfunction.
