Description
Viewed from superior aspect into the cranial cavity, the internal cranial base is opened to expose the trigeminal nerve (CN V) and its gasserian (trigeminal) ganglion resting in Meckel’s cave on the anterolateral surface of the petrous temporal bone. From the ganglion, V1 courses anteriorly toward the superior orbital fissure, V2 runs anteromedially toward the foramen rotundum, and V3 descends toward the foramen ovale, with these branches arranged in a fan-like pattern across the middle cranial fossa. The foramina of the sphenoid and temporal bones frame the pathways, while the foramen magnum lies centrally and posteriorly as the dominant landmark of the occipital bone. Orientation is immediate. Skull base neuroanatomy is where small spatial errors become clinical mistakes, and this superior perspective maps CN V branches to the exact bony exits targeted in procedures. Percutaneous trigeminal rhizotomy, balloon compression, or glycerol injection all aim to reach the trigeminal cistern via the foramen ovale, so the relationship between V3, the foramen ovale, and the adjacent petrous ridge matters in a way cross-sectional head diagrams often blur. The ganglion’s position at the petrous apex also helps explain why petrous apicitis (Gradenigo syndrome) can present with trigeminal distribution facial pain alongside abducens palsy.
