Description
Viewed from the superior aspect into the cranial cavity, the calvaria removed, the skull base is rendered with the anterior, middle, and posterior cranial fossae and their major foramina. Yellow cranial nerves course from the brainstem toward the internal acoustic meatus, jugular foramen, hypoglossal canal, and foramen ovale, while the trigeminal nerve is traced to its divisions along the lateral wall of the cavernous sinus. Medial to V3, the meningeal branch of the maxillary nerve (V2), classically the nervus spinosus, tracks posteriorly with the middle meningeal artery to reach the dura mater of the middle cranial fossa via the foramen spinosum. Blue vessels interlace with these neural pathways along the endocranial surface. Spatial relationships read cleanly. That V2 meningeal branch matters because it explains why dural irritation can refer pain into the maxillary distribution and why vascular pathology in the middle meningeal groove can be clinically loud. Epidural hematoma from middle meningeal artery rupture at the pterion sits on the same dural territory innervated by nervus spinosus, a useful correlation when teaching headache patterns versus focal neurologic deficit. Surgeons navigating the infratemporal fossa or elevating the dura from the middle cranial fossa also use the foramen spinosum, foramen ovale, and nearby V3 landmarks to avoid iatrogenic neural injury and to identify the artery early.
