Description
Centered within the lumbar column, the L3 vertebra is isolated in blue between the L2 vertebra superiorly and L4 inferiorly, separated by the L2 to L3 and L3 to L4 intervertebral discs. Its large vertebral body sits anterior to the vertebral arch, while the pedicles and laminae frame the vertebral foramen posteriorly. Laterally, the transverse processes project from the junction of pedicle and lamina; posteriorly, the spinous process extends in the midline. On the posterior elements, the mammillary processes rise from the superior articular processes, and the accessory processes lie at the posterior base of each transverse process. Clear landmarks. L3 is a common reference level when teaching lumbar biomechanics because its posterior elements are where facet joint orientation, pars interarticularis integrity, and deep paraspinal attachments converge. The mammillary processes provide attachment for multifidus and other short segmental stabilizers, while the accessory process relates to the longissimus thoracis insertion, so this posterior emphasis supports discussions of facet-mediated low back pain and segmental instability. In operative planning, identifying the L3 posterior bony contours helps with midline posterior approaches for laminectomy or pedicle screw trajectory, and it pairs well with explaining why L3 to L4 is a frequent level for degenerative stenosis.
