Description

Arising in the right lower quadrant, the cecum forms the initial, blind-ended pouch of the large intestine, positioned inferior to the terminal ileum and continuous superiorly with the ascending colon at the ileocecal junction. A narrow vermiform appendix is implied on the posteromedial aspect of the cecum, near the convergence of the taeniae coli. Distally, the colon transitions through the sigmoid colon, then continues into the rectum within the pelvis, where the haustra and taeniae coli fade as the rectal wall becomes more uniform. A cut section exposes the mucosa, submucosa, and muscularis externa beneath a glossy serosal surface. Orientation matters here because the cecum anchors right lower quadrant differential diagnosis and operative planning, from appendicitis to cecal volvulus and ileocecal intussusception. The relationship between the taeniae coli and the appendix is a practical landmark, surgeons follow the taeniae to the appendiceal base when the appendix is retrocecal or otherwise hard to find.
