Description

Opening into the left ventricle, the closed mitral (bicuspid) valve sits between the left atrium superiorly and the ventricular cavity inferiorly, with its anterior (aortic) and posterior leaflets coapting along a curved line to seal the left atrioventricular orifice. From the atrial side, the annulus fibrosus forms a pale, fibrous rim, while the leaflet free margins converge centrally where regurgitant flow would otherwise pass. Chordae tendineae extend from the leaflet edges and undersurface toward the anterolateral and posteromedial papillary muscles in the left ventricle, aligning the subvalvular apparatus along the ventricular long axis. Small endocardial vessels and trabecular contours frame the valvular complex. Coaptation in systole is the whole story. Loss of leaflet apposition from myxomatous degeneration and prolapse, rupture of chordae after infective endocarditis, or papillary muscle dysfunction following an inferior myocardial infarction (often affecting the posteromedial papillary muscle supplied by the right coronary artery) produces mitral regurgitation with a characteristic systolic murmur and left atrial volume overload. This interior view also supports procedural planning, because edge-to-edge repair (MitraClip) and surgical annuloplasty depend on understanding leaflet scallops, annular geometry, and the tethering vectors of the chordae.
