Primary and secondary chordae tendineae radiate distally from both leaflets to the anterolateral and posteromedial papillary muscles of the left ventricle, which project from the ventricular wall beneath the valve plane. Trabeculated ventricular myocardium lies inferior to the leaflet edges, while the smooth endocardial surface transitions proximally toward the left atrium. Diastolic opening matters because this is the functional geometry that governs transmitral flow and sets up coaptation for systole, and the atrial view makes the commissures, leaflet free margins, and chordal fan easier to read than a ventricular approach. Mitral valve prolapse and flail leaflet from ruptured chordae tendineae typically declare themselves at the free edge, and this perspective helps correlate leaflet scallops (P1 to P3) with regurgitant jets on transesophageal echocardiography.
