Description
Viewed from within the left ventricular outflow tract, the aortic valve sits superior to the left ventricle at the junction with the ascending aorta, its three semilunar cusps parted in systole to form a central, triangular orifice. The free margins of the right, left, and posterior (noncoronary) cusps bow slightly toward the aortic lumen, while their bases anchor to the fibrous aortic annulus. Just distal to the leaflet attachments, the aortic sinuses (sinuses of Valsalva) expand the root, and the coronary ostia arise from the right and left aortic sinuses. Anterior and inferior to the valve plane, the interventricular septum contributes to the outflow tract, and the anterior (mitral) leaflet lies posteroinferior as part of the aortomitral continuity. An endoluminal look at an open semilunar valve clarifies why cusp geometry matters: even minor thickening or commissural fusion narrows the effective orifice area and drives a high-velocity jet in aortic stenosis, classically producing concentric left ventricular hypertrophy. The same relationships guide transcatheter aortic valve replacement planning, where the annulus, sinuses, and coronary ostia height determine valve sizing and help avoid coronary obstruction. Regurgitant lesions read differently in this orientation, with poor cusp coaptation at the nodules of Arantius and along the commissures explaining central versus eccentric backflow.
