Description
Anterior abdominal viscera are rendered within a semi-transparent thoracoabdominal skeleton, with the lower rib cage superiorly and the pelvic ring inferiorly framing the cavity. The esophagus descends in the midline to the stomach, whose fundus lies under the left hemidiaphragm while the pylorus turns toward the right upper quadrant. Occupying the right hypochondrium, the liver overlies the stomach and proximal duodenum, with the gallbladder tucked on its visceral (inferior) surface; centrally and inferiorly, coiled jejunoileal loops fill most of the abdomen and are bordered peripherally by the colon. Spatial orientation is the teaching point. This arrangement clarifies why epigastric pain can reflect gastric pathology, right upper quadrant tenderness can suggest cholecystitis, and why the hepatobiliary system and proximal foregut are approached together in open and laparoscopic surgery (for example, during cholecystectomy where the cystic duct and artery are sought at the liver’s inferior margin). The relationship of the stomach to the liver and the surrounding bowel also helps explain patterns of distension, obstruction, and referred discomfort during gastritis, peptic ulcer disease, or ileus. Clear landmarks matter.
