Description
Presented from a posterior approach in the male abdomen, the stomach sits in the left upper quadrant with the esophagus entering superiorly, while the liver occupies the right upper quadrant with the gallbladder tucked on its visceral surface. Deep to these, the pancreas spans transversely across the posterior abdominal wall, and the C-shaped duodenum wraps around its head before transitioning into jejunal loops that coil centrally. The ascending and descending colon track vertically along the lateral margins, with the transverse colon crossing anterior to the small bowel and the rectum continuing inferiorly in the midline toward the anal canal. Retroperitoneal relationships read clearly against the vertebral column. Posterior viewing matters when you need to teach fixation and mobility of the gut. The second through fourth parts of the duodenum and the pancreas (except the tail) are retroperitoneal, so they stay pinned to the posterior abdominal wall during surgical exposure, blunt dissection, and trauma assessment, unlike the more mobile stomach and jejunoileal mesentery. This perspective also supports explanations of why inflammation or hemorrhage from pancreatitis can track posteriorly and why mobilization maneuvers such as a Kocher maneuver for the duodenum or medial mobilization of the descending colon are required to access posterior structures. It is a surgical anatomy view. Use this asset for gross anatomy and GI blocks when teaching peritoneal reflections, mesenteries, and the retroperitoneal portions of the gastrointestinal tract, and for operative atlases covering pancreaticoduodenal surgery, colectomy planes, and posterior approaches to the upper abdomen.
