Dorsal motor nucleus of the vagus (nucleus dorsalis nervi vagi) occupies the posteromedial medulla oblongata as a longitudinal column of preganglionic parasympathetic neurons. In this fixed dorsal brainstem perspective, it lies deep to the vagal trigone (trigonum nervi vagi) on the floor of the fourth ventricle, positioned inferior and slightly lateral to the hypoglossal trigone and medial to the nucleus of the solitary tract. A thin layer of ependyma and dorsal medullary parenchyma separates the nucleus from the ventricular surface. Orientation is unambiguous. This nucleus is the medullary origin of general visceral efferent fibers carried in cranial nerve X, and its topography within the dorsal vagal complex frames how clinicians think about autonomic findings in medullary disease. Lesions that involve the dorsomedial medulla, including infarction from vertebral or posterior inferior cerebellar artery territories, can interrupt vagal outflow and contribute to gastroparesis, impaired baroreflex responses, and cardiorespiratory instability, often alongside neighboring involvement of the nucleus tractus solitarius, area postrema, and adjacent reticular formation.
