Description
Seen from the lateral aspect of an adult male foot beneath the skin, the extensor hallucis brevis lies on the dorsum, arising from the superolateral surface of the calcaneus and the adjacent inferior extensor retinaculum region. Its short muscle belly sits anterior to the calcaneus and talus, superficial to the tarsal bones, and blends medially with the extensor digitorum brevis as it courses toward the first ray. A distinct tendon runs distally toward the proximal phalanx of the hallux, paralleling the longer extensor hallucis longus tendon and crossing the tarsometatarsal region over the cuneiform-metatarsal complex. Joint surfaces of the ankle and midfoot are delineated, with the metatarsals and phalanges aligned distally. Functionally, extensor hallucis brevis augments metatarsophalangeal extension of the great toe and helps refine toe clearance during swing phase, but its real teaching value is as a reliable dorsal landmark for neurovascular anatomy. The deep fibular (peroneal) nerve and dorsalis pedis artery travel on the dorsum of the foot between extensor hallucis longus and extensor digitorum longus tendons, immediately deep to the extensor digitorum brevis and its hallux slip, so swelling, laceration, or iatrogenic dissection in this corridor can produce first web-space sensory loss or compromise pedal pulses. Small muscle. Big implications. Use this lateral, subcutaneous rendering in gross anatomy and lower-limb modules to orient students to the calcaneal origin of the extensor digitorum brevis complex, and in sports medicine or foot and ankle surgery materials to support discussions of dorsal approaches, tendon injury, and dorsalis pedis palpation landmarks. It also reads well in radiology education when paired with ultrasound or MRI of the dorsolateral midfoot and anterior tarsal tunnel region.
