Description
Across the dorsum of the male foot, the extensor digitorum brevis (EDB) lies superficial to the dorsal tarsal region, arising from the superolateral calcaneus and the adjacent inferior extensor retinaculum. Its fleshy belly sits lateral to the tendon of extensor hallucis longus and overlies the calcaneocuboid area, while its tendinous slips course distally and medially toward the bases of the proximal phalanges of toes 2 to 4, blending with the extensor digitorum longus expansions. Medial to the EDB, the extensor hallucis brevis is often contiguous, running toward the proximal phalanx of the hallux. Clear bony context includes the talus and calcaneus proximally, the cuboid and cuneiforms midfoot, and the metatarsals and phalanges distally. EDB anatomy matters because it sits directly over the neurovascular corridor of the dorsum: the dorsalis pedis artery and deep fibular (peroneal) nerve traverse nearby between extensor hallucis longus and extensor digitorum longus before supplying the EDB and the first dorsal web space. This is a tight neighborhood. Pain or paresthesia in the first interdigital cleft can reflect deep fibular nerve entrapment under the extensor retinaculum, and the EDB belly is a practical surface landmark when locating a dorsalis pedis pulse or planning a dorsal approach to the tarsometatarsal joints.
