Description
Positioned on the dorsum of the foot, the extensor digitorum brevis (EDB) appears as a short, fleshy muscle belly arising from the superolateral calcaneus and lying deep to the extensor retinacula and the long extensor tendons. Its slips course anteriorly and medially toward the toes, joining the extensor expansions on digits 2 to 4 alongside the extensor digitorum longus tendons, while extensor hallucis brevis runs medial to the EDB toward the proximal phalanx of the hallux. Lateral in the field, the EDB sits over the tarsal bones and sinus tarsi region, with the ankle-level tendons from the anterior compartment passing distally toward the metatarsals and phalanges. Clear topography. This dorsal perspective matters because the EDB is a practical landmark for the deep fibular (peroneal) nerve and dorsalis pedis artery as they traverse the anterior ankle and dorsum, and because focal pain and swelling here often points you toward extensor tendinopathy or sinus tarsi pathology rather than midfoot joint disease. Clinicians also use EDB bulk as a quick bedside proxy for deep fibular nerve function in L5 radiculopathy or anterior tarsal tunnel syndrome, where weakness of toe extension may accompany dorsal foot paresthesia.
