Arising from the superolateral dorsum of the calcaneus, the extensor digitorum brevis forms a small muscular mass on the dorsum of the foot, lying deep to the skin and superficial fascia and lateral to the tendon of extensor hallucis longus. From its fleshy bellies, tendinous slips course distally and medially toward the dorsal aspects of the lateral four digits, blending with the extensor expansion of toes II to IV and sitting adjacent to the long extensor tendons of extensor digitorum longus. The muscle belly sits anterior to the calcaneus and inferior to the anterior ankle region, with the dorsalis pedis artery and deep fibular (peroneal) nerve running nearby on the dorsum. Palpation and exposure of extensor digitorum brevis matter in dorsal foot trauma and in operative approaches that use a dorsal incision, where swelling can obscure tendon landmarks and where the deep fibular nerve is at risk near the first intermetatarsal space.
