Description
Beneath translucent anterior skin of the distal leg and dorsum of the foot, the extensor digitorum longus (EDL) belly descends from the lateral condyle of the tibia and anterior fibula, becoming four tendons that run distally across the anterior ankle. At the level of the tibiotalar joint, those tendons pass deep to the superior and inferior extensor retinacula, then diverge over the dorsal midfoot toward toes two through five, where they continue into the extensor expansions on the phalanges. Medially, the tibialis anterior tendon parallels the EDL toward the medial cuneiform and base of the first metatarsal, while the extensor hallucis longus tendon occupies the interval between them, a corridor that also carries the anterior tibial artery as it becomes dorsalis pedis and the deep fibular (peroneal) nerve. Clinically, this anterior relationship is the one you want when teaching the anterior compartment and when explaining foot drop from common fibular nerve palsy, since EDL shares innervation (deep fibular nerve) and loses toe extension early. Overuse tenosynovitis of the EDL can produce dorsal ankle pain and crepitus under the extensor retinaculum, and the same confined space becomes unforgiving in acute anterior compartment syndrome. A clean landmark. Teaching and publishing use cases are straightforward: lower limb anatomy labs (muscle compartments and ankle retinacula), podiatry and sports medicine modules on dorsal foot pain, and operative or imaging primers that map dorsalis pedis access in relation to the EHL and EDL tendons.
