Description
Arising from the lateral epicondyle of the humerus and adjacent antebrachial fascia, the extensor digitorum occupies the central part of the posterior forearm extensor mass, with its muscle belly lying superficial to the deeper extensors and sending tendons distally toward the dorsum of digits II to V. From an anterior perspective, the radius appears lateral and the ulna medial in anatomical position, while the extensor digitorum is read through its relationship to these bony columns and to the elbow and radiocarpal joints. Distally, the tendons course beneath the extensor retinaculum, then expand into the dorsal digital expansions over the metacarpophalangeal and interphalangeal joints. Proximal to distal continuity is clear. That tendon pathway is where clinicians focus when evaluating lateral elbow pain and dorsal wrist complaints. Common extensor tendinopathy (lateral epicondylitis) typically involves the extensor carpi radialis brevis, but extensor digitorum shares the common extensor origin and is frequently tender with resisted finger extension, a useful differentiator during exam. In the hand, the extensor digitorum’s contribution to the extensor hood explains why sagittal band rupture can produce extensor tendon subluxation at the metacarpophalangeal joint, and why proximal interphalangeal joint imbalance can evolve into boutonniere deformity after central slip injury.
