Description
Running along the posterior compartment of the forearm, the extensor digitorum muscle occupies a broad, superficial plane from the lateral epicondyle region of the humerus to the dorsum of the hand. Proximally it lies lateral to extensor carpi ulnaris and medial to extensor carpi radialis longus and brevis, then narrows distally into multiple tendons that pass deep to the extensor retinaculum toward the metacarpals and phalanges. The radius sits laterally and the ulna medially in anatomical position, framing the extensor mass as it crosses the elbow joint and continues over the dorsal wrist. Bony landmarks stay readable. This posterior view matters because extensor digitorum is the main extrinsic extensor for digits 2 through 5, and its tendons form the dorsal digital expansions that distribute force across the metacarpophalangeal and interphalangeal joints. Clinically, the distal tendons and their synovial sheaths are common sites of tenosynovitis and can be compromised by dorsal lacerations at the wrist or metacarpal level, where tendon continuity and compartment relationships guide repair. Lateral epicondylitis also centers on the common extensor origin, so seeing the muscle’s proximal attachment and line of pull helps connect exam findings to anatomy.
