Description
Running down the anterior compartment of the leg, the extensor hallucis longus (EHL) becomes a slender tendon that crosses the tibiotalar joint anteriorly and continues onto the dorsum of the foot toward the distal phalanx of the hallux. Medial to it, the tibialis anterior tendon angles toward the medial cuneiform and base of the first metatarsal, while the extensor digitorum longus tendons lie more laterally as they fan toward the lesser toes. At the ankle, the EHL tendon passes deep to the superior and inferior extensor retinacula, with the tibia and dorsal tarsal bones forming the deep osseous backdrop. Superficial skin and subcutaneous tissue are retained to keep the course realistic. Clinically, this anterior view is the one you want when teaching dorsiflexion and hallux extension mechanics because the EHL tendon is a reliable surface landmark between tibialis anterior and extensor digitorum longus, and it closely neighbors the dorsalis pedis artery and deep fibular (peroneal) nerve on the dorsum of the foot. Lacerations over the anterior ankle often sever EHL and leave the patient unable to extend the great toe, while anterior ankle tendinopathy and postsurgical adhesions can limit glide beneath the extensor retinacula. Small structure. Big functional consequence.
