Description
Anteriorly, the male lower leg is presented with the tibia positioned medially and the fibula laterally, with the bony contours continuing into the tarsals, metatarsals, and phalanges of the foot. The extensor hallucis longus arises from the anterior surface of the fibula and adjacent interosseous membrane, running inferiorly in the anterior compartment between tibialis anterior (medial) and extensor digitorum longus (lateral) before narrowing into a tendon. Distally, its tendon passes deep to the extensor retinacula at the anterior ankle and continues onto the dorsum of the foot to insert on the dorsal aspect of the base of the distal phalanx of the hallux. Clean, teachable anatomy. Clinically, this anterior perspective clarifies why extensor hallucis longus function is often tested during L5 motor assessment, with resisted great toe extension serving as a quick bedside correlate when radiculopathy is suspected. The relationship of the tendon to the superior and inferior extensor retinacula also matters in anterior ankle approaches and in extensor tendon tenosynovitis, where pain tracks along the dorsum of the foot and can be localized by stressing the hallux in extension. It also helps explain early dorsiflexion weakness patterns in common fibular (peroneal) nerve injury at the fibular neck, where anterior compartment muscles can be affected. Ideal use includes lower limb dissection manuals, neuroanatomy teaching on myotomes and peripheral nerve lesions, and surgical education materials covering the anterior ankle and dorsum of foot tendons.
