Description
Running along the anterolateral shin, the extensor digitorum longus (EDL) occupies the anterior compartment just lateral to tibialis anterior and anterior to the distal fibula, its muscle belly tapering into multiple tendons that descend toward the dorsum of the foot. Distally, the EDL tendons pass deep to the superior and inferior extensor retinacula at the anterior ankle, then course over the metatarsals to the lateral four toes, where they contribute to the dorsal digital expansions. The tibia sits medially as the dominant bony landmark, with the fibula more lateral and posterior, and adjacent lateral-compartment muscle bulk (often the fibularis longus) providing a useful contrast in fiber direction and compartment boundaries. Skin and superficial fascia remain as a thin overlayer. Clear landmarks. Teaching EDL in an anterior, near-surface context matters because its tendons, the anterior tibial vessels, and the deep fibular (peroneal) nerve share tight real estate beneath the extensor retinacula, a region where lacerations, entrapment, or postoperative scarring can produce toe extension weakness and dorsal foot sensory symptoms. This view also clarifies why anterior compartment syndrome presents with pain on passive toe flexion and why foot drop patterns differ between deep fibular neuropathy and more proximal common fibular nerve injury at the fibular neck. For surgeons, it frames safe interval planning for anterior ankle arthroscopy and for tendon transfer procedures that borrow EDL slips to rebalance deformity without sacrificing all digital extension. Use this plate in lower-limb gross anatomy labs, kinesiology modules on toe extension and ankle dorsiflexion, or podiatry and sports-medicine teaching files on anterior ankle pain and tendon injury patterns.
