Description
Across the dorsal aspect of the male foot, the extensor hallucis longus tendon runs superficially from the anterior ankle toward the distal phalanx of the hallux, crossing the talus and tarsometatarsal region before centering on the first ray. A thin tendon sheath tracks its length beneath the skin, bordered laterally by the tendons of extensor digitorum longus and medially by tibialis anterior as they pass deep to the superior and inferior extensor retinacula. Beneath the translucent soft-tissue layer, the tarsals (talus, calcaneus, navicular, cuboid, and cuneiforms), metatarsals, and phalanges align in dorsal view, with articular cartilage suggested at the ankle, midtarsal, and metatarsophalangeal joints. Skin-level landmarks remain readable. Clinically, the extensor hallucis longus is the most accessible tendon for testing deep fibular (peroneal) nerve function, since isolated weakness of hallux extension helps distinguish L5 radiculopathy from common fibular neuropathy when paired with tibialis anterior and extensor digitorum longus exam. On the dorsum, the tendon also brackets the dorsalis pedis artery, which typically lies just lateral to extensor hallucis longus, a practical relationship when palpating pulses or planning incisions for dorsal approaches to the first metatarsophalangeal joint. Sharp lacerations and crush injuries on the dorsum frequently violate this sheath and produce extensor lag of the hallux.
