Description
Running along the anterior compartment of the leg, the extensor hallucis longus muscle narrows into a cord-like tendon that crosses the anterior ankle deep to the superior and inferior extensor retinacula, then tracks distally over the dorsum of the foot to the dorsal aspect of the hallux. Lateral to its proximal muscle belly lies the fibula, while the tibia sits more medially at the shin, framing the tendon’s course toward the first metatarsal and proximal phalanx. Adjacent dorsal extensor tendons to the lesser toes are suggested nearby, with the extensor hallucis longus tendon typically positioned between tibialis anterior medially and extensor digitorum longus laterally. A red vessel consistent with the anterior tibial artery continuing as dorsalis pedis runs in close relationship, a key surface landmark on the dorsum. That spatial arrangement matters at the ankle and midfoot, where the extensor hallucis longus tendon guides toe extension and helps stabilize the hallux during swing phase. Injury patterns cluster here. Tendinopathy and laceration can follow anterior ankle trauma, and surgeons often use the tendon as a reference when identifying the dorsalis pedis artery during dorsal approaches to the midfoot or first metatarsophalangeal joint. The tight osteofibrous tunnel under the extensor retinacula also helps explain pain with resisted hallux extension when retinacular irritation or tenosynovitis develops.
