Proximally, the sheath tracks the tendon as it crosses the tibiotalar region, then continues distally toward the dorsum of the hallux, where the tendon lies medial to the extensor digitorum longus tendons. A thin synovial membrane lines the fibrous covering, forming a low-friction tunnel that keeps the tendon apposed to the underlying talus, navicular, and dorsal cuneiform region during dorsiflexion and toe extension.
