Medial anatomy of the male hindfoot is centered on the flexor hallucis longus tendon as it courses posterior to the talus and along the plantar aspect of the foot toward the hallux. A synovial tendon sheath surrounds the tendon, forming a lubricated tunnel deep to the medial malleolar region and adjacent to the sustentaculum tali of the calcaneus, before continuing distally toward the great toe. Medially, this relationship places the sheath near the tarsal tunnel contents, with the tendon lying posterior and inferior to the ankle mortise as it approaches the plantar foot. Clean, deep pathways dominate the composition. Clinically, the flexor hallucis longus sheath is a common site of stenosing tenosynovitis in dancers and runners, where repetitive plantarflexion causes pain and triggering along the posteromedial ankle and plantar hallux. This view also supports surgical planning for posteromedial endoscopic release or debridement, where identifying the tendon’s gliding corridor helps avoid iatrogenic injury to the tibial nerve and posterior tibial artery within the tarsal tunnel.
