Medial hindfoot anatomy is presented with the distal tibia and fibula superiorly, the talus seated between them, and the calcaneus forming the posterior-inferior pillar of the ankle and subtalar joints. Along the posteromedial aspect, the flexor digitorum longus tendon descends deep to the flexor retinaculum, enclosed by its synovial tendon sheath as it traverses the tarsal tunnel behind the medial malleolus. Distal to the tunnel, the tendon courses plantarward toward the midfoot and forefoot, where it will divide to the lateral four toes. Tight quarters. Clinical attention centers on the tarsal tunnel, where the tendon sheath shares space with the posterior tibial tendon, flexor hallucis longus tendon, and the posterior tibial neurovascular bundle.
