Description
Medial hindfoot anatomy is centered on the flexor digitorum longus tendon as it courses posterior to the medial malleolus of the tibia, enclosed by a synovial tendon sheath. The sheath forms a smooth tubular covering along the retromalleolar groove, positioned deep to the flexor retinaculum and adjacent to the tarsal tunnel contents. Just anterior lies the tibialis posterior tendon and its sheath, while the flexor hallucis longus tendon runs more posterior and lateral toward the sustentaculum tali. Tight quarters. This medial view matters because the retromalleolar segment of flexor digitorum longus is a common site for friction-related tenosynovitis and for tendon entrapment when the flexor retinaculum thickens or scarring follows ankle trauma. Surgical exposure for tarsal tunnel release or medial malleolar approaches to the hindfoot relies on knowing which sheath you are entering, since opening the wrong synovial compartment can increase postoperative adhesions and stiffness in toe flexion.
