Description
Medial ankle anatomy is centered on the tibialis posterior tendon as it descends in the deep posterior compartment, curves posterior to the medial malleolus, and continues distally toward its insertion on the navicular tuberosity with expansions to the cuneiforms and metatarsal bases. A synovial tendon sheath surrounds the tendon within the tarsal tunnel, deep to the flexor retinaculum, and lies immediately posterior to the medial malleolus. Anterior to it runs the tendon of flexor digitorum longus, while the flexor hallucis longus tendon sits more posterior and lateral as it tracks toward the sustentaculum tali. Medially, the sheath relates to subcutaneous tissues along the malleolar region; laterally, it borders the neurovascular bundle. Clinically, this view maps the segment where tibialis posterior tenosynovitis most often develops, with sheath thickening and fluid distention producing posteromedial ankle pain and crepitus during resisted inversion. It also clarifies the relationship to the posterior tibial artery and tibial nerve inside the tarsal tunnel, a practical point when distinguishing tendon sheath pathology from tarsal tunnel syndrome or planning ultrasound guided sheath injection.
