Medial anatomy of the anterior ankle is centered on the tibialis anterior tendon as it descends from the anterior compartment of the leg, passes anterior to the tibiotalar joint, and continues distally toward its insertion on the medial cuneiform and base of the first metatarsal. A synovial tendon sheath surrounds the tendon at the level of the ankle, with a parietal layer lining the fibrous sheath and a visceral layer investing the tendon to reduce friction. The sheath lies deep to the extensor retinacula and immediately anterior to the medial malleolus, with the tendon positioned medial to extensor hallucis longus and anteromedial to the talar neck and navicular. Clinically, this view helps clarify why tibialis anterior tenosynovitis presents as pain and swelling along the anteromedial ankle, often aggravated by dorsiflexion and resisted inversion. The relationship to the extensor retinaculum matters: retinacular thickening or post-traumatic scarring can restrict tendon excursion and mimic joint pathology. It also frames common imaging targets, since ultrasound and MRI frequently demonstrate sheath distention, synovial hypertrophy, and fluid tracking proximal or distal to the ankle joint capsule. Use this asset for lower-limb anatomy teaching in gross anatomy, kinesiology, and podiatric medicine modules where students confuse tendon sheath pathology with ankle synovitis.
