Running along the anterior ankle and dorsum of the foot, the extensor hallucis longus tendon is shown within its synovial tendon sheath as it passes deep to the extensor retinacula and continues distally toward the hallux. Medial to the tendon lie the tibialis anterior tendon and the medial malleolus of the tibia, while the tendon to extensor digitorum longus sits more laterally as it crosses the ankle mortise formed by tibia, fibula, and talus. Beneath this gliding tunnel, the dorsal surfaces of the talus, navicular, cuneiforms, and first metatarsal form the bony contour that the sheath must negotiate. Clinically, this anterior view is the one you want when teaching or planning around extensor hallucis longus tenosynovitis and stenosing irritation beneath the superior or inferior extensor retinaculum, where pain often localizes just anterior to the ankle joint and worsens with resisted hallux dorsiflexion. The sheath also sits in a crowded neighborhood with the anterior tibial artery and deep fibular (peroneal) nerve in the anterior compartment, so swelling, scarring, or iatrogenic injury during anterior ankle arthroscopy portals or dorsal midfoot procedures can have outsized consequences.
