Description
Encircling the anal canal at the distal end of the rectum, the external anal sphincter forms a thick ring of skeletal muscle within the male perineum. Its subcutaneous part lies most inferior and superficial at the anal verge, while the superficial and deep parts extend superiorly to blend with the perineal body anteriorly and abut the puborectalis sling of the levator ani posteriorly. Laterally, the muscle mass relates to the ischioanal (ischiorectal) fossae, with the pudendal canal and internal pudendal vessels positioned more lateral along the obturator internus fascia. Layered deep to it, the internal anal sphincter contributes a separate smooth muscle cuff. A clear muscular boundary. Voluntary continence depends on this muscle’s tonic activity and rapid recruitment, mediated primarily by the inferior rectal branches of the pudendal nerve (S2 to S4), which travel across the ischioanal fossa toward the sphincter complex. This anatomy anchors common clinical problems: obstetric-style perineal tears are less relevant in the male, but iatrogenic sphincter injury during fistulotomy or hemorrhoidectomy can produce postoperative fecal incontinence, and perianal abscesses often track through the intersphincteric plane into the ischioanal fossa. In anorectal physiology, distinguishing external sphincter dysfunction from internal sphincter hypotonia helps interpret manometry and guides operative planning.
