Description
Seen from a superior perspective in the male perineum, the external anal sphincter encircles the anal canal as a striated muscular cuff with deep fibers interdigitating with the puborectalis and adjacent levator ani. Medially, its fibers meet at the anococcygeal raphe posteriorly and blend anteriorly toward the perineal body, where the sphincter complex relates to the midline fibromuscular node between the anal canal and urogenital structures. Laterally, the sphincter lies just medial to the ischioanal fossa fat and the obturator internus fascia that forms the lateral pelvic wall. Orientation is everything. This superior view clarifies how the external anal sphincter and levator ani function as a single continence unit, a relationship that becomes clinically apparent when pelvic floor integrity is lost after obstetric injury, prostatectomy, or pelvic radiotherapy, even though the illustrated subject is male.
