Description
Across the lateral thoracoabdominal wall, the external oblique muscle is presented with its classic inferomedial fiber direction, arising from the external surfaces of ribs 5 to 12 and sweeping anteriorly toward the linea alba. Superiorly, slips interdigitate with the serratus anterior and lie superficial to the intercostal musculature spanning adjacent ribs, while posterior fibers approach the thoracolumbar region. Anteroinferiorly, the muscle transitions into a broad external oblique aponeurosis that contributes to the anterior rectus sheath and forms the inguinal ligament between the anterior superior iliac spine and the pubic tubercle. Clean landmarks. This obliquity of fiber orientation is the teaching point and the clinical one. The inferomedial external oblique fibers oppose the superomedial internal oblique during trunk rotation and provide the superficial layer you split (not cut) in an open inguinal hernia repair to expose the inguinal canal and spermatic cord, with the superficial inguinal ring represented as a defect in the aponeurosis. Understanding where the aponeurosis fans medially also clarifies why lateral abdominal wall injuries and incisions can weaken the anterior abdominal wall and predispose to incisional hernia.
