Description
Facial nucleus (nucleus nervi facialis) occupies the caudal pontine tegmentum as an ovoid motor cell column positioned ventrolateral to the abducens nucleus and just deep to the floor of the fourth ventricle. In an axial brainstem orientation, its medial border lies near the midline tegmental structures, while laterally it approaches the pontine reticular formation and the emerging rootlets of the facial nerve at the cerebellopontine angle. Fibers from this nucleus course dorsomedially as the internal genu of the facial nerve, looping around the abducens nucleus to create the facial colliculus on the dorsal pons, then turn ventrolaterally toward the pontomedullary junction. Nearby landmarks you would expect in the same slice include the medial longitudinal fasciculus, central tegmental tract, and transverse pontine fibers anteriorly. Clinically, the facial nucleus is a key localization point for lower motor neuron facial weakness, because lesions here produce ipsilateral paralysis of the entire hemiface (including forehead), often with reduced efferent limb of the corneal blink reflex via orbicularis oculi. Small paramedian pontine infarcts, demyelinating plaques, or intrinsic pontine tumors can involve the nucleus itself or the intrapontine fascicles, and the adjacency to the abducens nucleus helps explain combined facial palsy with horizontal gaze palsy in dorsal pontine syndromes. Tight separation of nuclei and fiber loops in the tegmentum makes spatial relationships the teaching objective. One level tells the story.
