Description
Centered on the medial thigh, the adductor longus is isolated in blue against the surrounding anterior compartment musculature, with its proximal tendon attaching on the anterior body of the pubis just inferior to the pubic crest and adjacent to the pubic symphysis. From that origin it courses inferolaterally along the inner thigh, lying superficial to adductor brevis and anterior to adductor magnus, and forming a clear medial border to the femoral triangle with sartorius laterally and the inguinal region superiorly. Distally, its aponeurotic insertion aligns with the middle third of the linea aspera on the posteromedial femoral shaft, while the knee is included as a spatial reference where the femur meets the tibia and patellar tendon and collateral ligamentous contours are suggested in white. Orientation is clear. For groin pain workups, this anatomy matters because the adductor longus origin is a common site of acute strain and chronic tendinopathy in kicking sports and cutting maneuvers, often tender along the pubic attachment and exacerbated by resisted adduction. The relationship of the muscle belly to the femoral vessels and adductor canal also frames why adductor tenderness can coexist with anterior hip symptoms, and why targeted physical exam, ultrasound-guided injection, or an obturator nerve block must respect nearby neurovascular structures. Surgical planning for adductor tenotomy or release in refractory athletic pubalgia also starts with accurate localization of the proximal footprint.
