Description
Running along the posterolateral cervical spine, the iliocostalis cervicis appears as the upper continuation of the iliocostalis column of the erector spinae, arising from the angles of the upper ribs (classically ribs 3 to 6) and inserting onto the posterior tubercles of the transverse processes of C4 to C6. Medial to it sits the longissimus cervicis, while the trapezius forms a superficial roof over the upper thorax and neck, framing deeper epaxial muscles around the midline spinous processes. Inferiorly the iliocostalis blends into the thoracolumbar fascia and the broader erector spinae mass; laterally it approaches the costal cage and the medial border of the scapula where the rhomboids and levator scapulae define the scapulothoracic contour. Clear segmental patterning of the posterior neck muscles helps you track vertebral levels. A clean posterior map. For clinicians, the iliocostalis cervicis matters because it is a common generator of cervical myofascial pain and spasm after whiplash, prolonged desk posture, or compensatory overuse when the trapezius is inhibited. Palpation and dry needling are safer and more reproducible when you understand how this muscle lies lateral to the spinous processes and deep to the superficial fascia, and how its line of pull can mimic facet-mediated pain at mid-cervical levels. The same relationships also inform posterior cervical approaches where muscle-splitting planes aim to preserve the dorsal rami and reduce postoperative denervation and atrophy.
