Description
Positioned along the posterolateral pelvic floor, the iliococcygeus forms the thinner, more aponeurotic portion of the levator ani as it spans from the tendinous arch of levator ani and the ischial spine toward the anococcygeal raphe and coccyx. Medial to it, the pubococcygeus sits closer to the midline and pelvic viscera, together creating a sling that supports the rectum and maintains the pelvic outlet. Portions of the bony pelvis, including the ilium and ischium, frame the muscle origins laterally, while the coccyx lies posterior and inferior as a distal attachment landmark. Fiber direction is clear. For teaching male pelvic anatomy, this view matters because iliococcygeus is often underappreciated in comparison with puborectalis, yet it contributes to the pelvic diaphragm’s lateral tensioning and helps resist descent of the pelvic floor during increases in intra-abdominal pressure. Clinically, levator ani avulsion and chronic pelvic floor weakness are discussed more often in obstetrics, but men present with pelvic pain syndromes and post-prostatectomy urinary incontinence where targeted pelvic floor rehabilitation depends on understanding which levator components can be recruited.
