Description
Posterior neck and upper thorax are rendered with the iliocostalis cervicis emphasized within the erector spinae column. Its muscle belly lies lateral to the longissimus cervicis and deep to the superficial trapezius, with fibers running superomedially from the angles of the upper ribs (classically ribs 3 to 6) toward the posterior tubercles of the transverse processes of C4 to C6. Medially, the cervical spinous processes and laminae provide bony reference, while neighboring posterior neck layers such as splenius cervicis, semispinalis cervicis, and levator scapulae establish depth and fiber direction around the cervicothoracic junction. Clinically, isolating iliocostalis cervicis in a posterior view helps clarify how lateral erector spinae components contribute to cervical extension and ipsilateral side-bending while stabilizing the lower cervical segments, a relationship that becomes obvious when you compare its line of pull with the more medial semispinalis group. Local overload and myofascial trigger points in the iliocostalis and adjacent longissimus can present as posterior neck pain after prolonged desk posture or whiplash-type acceleration injury, and the illustration’s bony landmarks support teaching palpation and differential diagnosis from levator scapulae strain.
