Description
Arising from the anterior rami of L1 within the lumbar plexus, the ilioinguinal nerve is traced along the posterior abdominal wall as it courses inferolaterally across the quadratus lumborum and toward the iliac crest. From a left lateral perspective, its trajectory is shown running parallel to, and just inferior to, the iliohypogastric nerve, a relationship that helps orient the two terminal branches of L1. Bony landmarks of the ribcage, thoracolumbar vertebrae, sacrum, and the left hemipelvis provide the scaffold for understanding where the nerve leaves the spine and approaches the inguinal region. Color-coded nerve lines contrast cleanly against the skeletal model. For inguinal anatomy, this is the nerve most often encountered unexpectedly. Ilioinguinal nerve entrapment or iatrogenic injury after open inguinal hernia repair, appendectomy via a lower abdominal incision, or Pfannenstiel-type approaches produces burning pain or hypoesthesia over the superomedial thigh and anterior scrotum in the male, with symptoms that can mimic hernia recurrence. Its close association with the iliohypogastric nerve and their shared L1 origin matters when planning targeted nerve blocks, mapping dermatomal symptoms, or explaining why a single suture line near the internal oblique can create disproportionate postoperative neuralgia.
