Description
Arising from the anterior ramus of L1 within the lumbar plexus, the iliohypogastric nerve courses inferolaterally across the anterior surface of quadratus lumborum and approaches the iliac crest, with the psoas major lying medial and the transversus abdominis lateral and anterior. A slender lateral cutaneous branch is shown emerging near the iliac crest and turning toward the posterolateral gluteal region, while a more anterior continuation would be expected to run between the internal oblique and transversus abdominis toward the suprapubic area. Bony landmarks in profile include the lower rib cage, lumbar vertebrae, sacrum, and ilium, supporting orientation along the thoracolumbar junction. Nerve trajectories are emphasized against the ribcage, spine, and pelvis. Recognition of the iliohypogastric nerve’s lateral cutaneous branch matters during lower abdominal wall exposure, because it is commonly encountered and injured in open appendectomy (McBurney muscle-splitting incision) and Pfannenstiel incisions for cesarean delivery or pelvic procedures. Injury or entrapment where the nerve pierces the internal oblique can produce burning pain or hypoesthesia over the posterolateral gluteal skin and the suprapubic region, and it can mimic L1 radiculopathy or ilioinguinal neuralgia. Small nerve, real consequences.
