Description
Posterolateral cervical anatomy is presented with the skin removed to expose the superficial and deep muscle planes of an adult male neck. The upper fibers of trapezius sweep superomedially from the shoulder girdle to the occipital region, while splenius capitis runs superolaterally toward the mastoid and nuchal lines, forming a muscular roof over deeper extensors. Along the anterolateral neck, sternocleidomastoid defines the lateral boundary of the posterior triangle, and the scalene group lies deep to it, descending toward the first and second ribs. Small suboccipital muscles, highlighted in red, sit immediately inferior to the occiput in the posterior cervical region. Small but clinically loud. That suboccipital layer, including rectus capitis posterior major and minor and the obliquus capitis muscles, is a common teaching focus for head and atlas-axis kinematics because it anchors motion at the atlanto-occipital and atlanto-axial joints while blending with the posterior cervical fascia. Tension, hypertrophy, or postural overload in these muscles is often implicated in cervicogenic headache patterns and may irritate the greater occipital nerve as it courses from C2 dorsal ramus through the semispinalis capitis toward the scalp. The relationship of sternocleidomastoid to the posterior triangle also frames where clinicians palpate and where surgeons plan incisions to avoid the spinal accessory nerve on its way to trapezius.
