Description
Pars oralis of the spinal trigeminal nucleus is visualized as an elongated column of gray matter in the dorsolateral brainstem, spanning the caudal pons and rostral medulla. Its position lies medial to the spinal trigeminal tract and lateral to the medial lemniscus region, with the nucleus hugging the posterolateral tegmentum rather than the ventral basis. Rostrocaudally, the column continues inferiorly toward pars interpolaris and pars caudalis, while superiorly it approaches the level where trigeminal sensory processing transitions toward pontine nuclei. Clinically, this segment of the trigeminal sensory complex is tied to facial and oral somatosensation, including afferent input from the teeth, oral mucosa, and perioral skin carried primarily via the trigeminal nerve (CN V). Lesions in the dorsolateral medulla or lower pons can compromise ipsilateral facial pain and temperature and may accompany Wallenberg syndrome when neighboring vestibular nuclei, nucleus ambiguus, and sympathetic pathways are involved.
