Description
Prepositus nucleus (nucleus prepositus hypoglossi) occupies the periventricular gray of the rostral medulla, forming a slender, elongated cell column along the dorsomedial tegmentum at the floor of the fourth ventricle. Medially it lies close to the hypoglossal nucleus and the midline ependymal surface, while laterally it approaches the medial vestibular nucleus and adjacent reticular formation. Its rostrocaudal extent parallels the ventricular margin near the obex region, with fiber pathways coursing nearby in the medial longitudinal fasciculus. Small structure, tight neighborhood. Gaze holding depends on this nucleus. Neurons in the prepositus complex integrate vestibular and ocular motor signals to help maintain eccentric eye position, so lesions in the dorsomedial medulla can present with gaze-evoked nystagmus, impaired smooth pursuit, or defective vestibulo-ocular reflex adaptation even when the abducens nucleus and vestibular nuclei appear structurally intact.
