Description
Temporal pole anatomy is centered on the anterior-most cortex of the temporal lobe, where the middle temporal gyrus and inferior temporal gyrus converge toward a blunt anterior apex. From a ventrolateral perspective, the pole sits inferior to the lateral (Sylvian) fissure and anterior to the temporal neocortex, with the superior temporal gyrus and temporal operculum positioned more superolaterally. Medially, the cortical surface curves toward the uncus and parahippocampal region, while deep to the pole lie the anterior temporal white matter and limbic structures that include the amygdala at the anteromedial temporal lobe. A compact landmark. Clinically, the temporal pole is a key surface landmark when localizing anterior temporal pathology and planning approaches that must respect adjacent language and memory networks. In anterior temporal lobectomy for drug-resistant temporal lobe epilepsy, surgeons often orient to the temporal pole to estimate the extent of neocortical resection while avoiding excessive superior extension toward the Sylvian fissure and peri-insular vessels. The region is also a common site of involvement in semantic variant primary progressive aphasia (semantic dementia) and can show early signal change in herpes simplex encephalitis, making a well-defined cortical boundary between middle and inferior temporal gyri useful for teaching and for figure-ground clarity in publications. Neuroanatomy and neuropsychology courses use this illustration to anchor discussions of temporal lobe surface gyri, limbic proximity, and clinicoradiologic correlation in epilepsy conference handouts and neurosurgical atlases.
