Description
Running longitudinally along the posterior aspect of the vertebral column, the posterior spinal vein is rendered as a slender blue vessel tracking near the midline over the dorsal elements of the thoracic vertebrae, with adjacent ribs shown in semi-transparent gray for orientation. Segmental connections extend laterally and slightly anteriorly toward the intervertebral foramina, where the vein communicates with radicular veins accompanying the spinal nerve roots. Posterior arches, spinous processes, and costovertebral relationships remain readable beneath the vascular highlight. Orientation is posterior and thoracic. Venous drainage of the spinal cord and its coverings is easy to mis-teach because it lacks valves and routes blood through a network that links intradural veins to the internal vertebral venous plexus and then to segmental systems. This posterior spinal vein to radicular vein pathway matters in the setting of spinal dural arteriovenous fistula, where arterialized venous outflow produces congestive myelopathy and symptoms may localize poorly despite a thoracic predominance. It also explains how infection or metastasis can spread through the vertebral venous system without a clear lymphatic pattern. A small vessel. Big implications.
