Description
Spanning the posterior neck and upper thorax, the trapezius forms a broad, superficial sheet from the occipital bone and superior nuchal line down along the nuchal ligament and spinous processes to about T12. Its fibers sweep laterally to insert on the lateral third of the clavicle, the acromion, and the spine of the scapula, creating the familiar trapezoid outline across the back and shoulder girdle. Superior (descending) fibers run inferolaterally toward the clavicle, middle (transverse) fibers pass laterally to the acromion and scapular spine, and inferior (ascending) fibers course superolaterally to the medial end of the scapular spine. The scapula sits deep to the muscle belly, with the thoracic spine medial and the deltoid more lateral at the shoulder contour. Palpation and surface anatomy of trapezius matter in both diagnosis and procedure.
