Description

Upper airway anatomy is foregrounded from the nasal cavities and choanae through the nasopharynx, oropharynx, and laryngopharynx into the laryngeal inlet. Inferior to the cricoid cartilage, the trachea descends in the midline anterior to the esophagus, then bifurcates at the carina into the right and left main bronchi that branch into lobar and segmental bronchi within each lung. Bronchial generations taper distally toward bronchioles and the peripheral gas exchange region, with the bronchial tree occupying the medial aspects of the lungs before spreading laterally toward the pleural surfaces. Orientation is anatomical, with superior structures of the nasal cavity leading caudally into the thorax. Airway continuity is the point. Clinically, this arrangement explains why aspirated material more often enters the right main bronchus, which is typically wider and more vertical than the left, and why carinal deviation can signal mediastinal shift or volume loss. The larynx and proximal trachea also anchor common airway procedures, from endotracheal intubation to surgical tracheostomy, where the thyroid isthmus and the level of the second to fourth tracheal rings guide safe access. Depth cues and selective emphasis on the luminal pathway support teaching of airflow direction, mucociliary clearance, and obstruction sites in asthma, bronchitis, and COPD.
