Description
Inferior articular process of the axis (C2) anchors the caudal zygapophysial articulation with C3, projecting inferiorly from the junction of the lamina and pedicle on the posterior arch. The inferior articular facet sits posterolateral to the vertebral body, just medial to the transverse process, and its smooth surface faces inferiorly with a slight anterior orientation toward the C2 to C3 facet joint. Adjacent bony landmarks commonly included in the same frame are the pars interarticularis, the base of the transverse process with its rounded tubercle, and the posterior elements of the vertebral arch. Bony contours are crisp. Facet anatomy at C2 matters because it defines the line of load transfer between the upper cervical spine and the subaxial cervical segment, and it also sets the corridor for posterior instrumentation. The relationship between the inferior articular process and the pars interarticularis is the teaching point for traumatic spondylolisthesis of the axis (hangman fracture), where fractures through the pars can destabilize C2 on C3 while leaving the vertebral body relatively intact. Pain physicians also reference this region when targeting the C2 to C3 facet joint and the third occipital nerve during diagnostic blocks for cervicogenic headache, so faithful surface orientation and edge definition help when explaining needle trajectories and joint line location.
