Description
Interventricular foramen (foramen of Monro) sits at the junction of the lateral ventricle and the third ventricle, bordered medially by the column of the fornix and laterally by the anterior thalamus. The opening lies just posterior to the anterior commissure region, with the septum pellucidum and body of the fornix superiorly and the head of the caudate nucleus forming the anterolateral ventricular wall in the same fixed field. Choroid plexus is typically seen approaching the aperture from the lateral ventricle and continuing into the roof of the third ventricle, with the thalamostriate vein and internal cerebral venous confluence expected nearby along the choroidal fissure. Small space, big consequences. Obstruction at this narrow passage is a classic cause of noncommunicating hydrocephalus, often presenting with unilateral or asymmetric dilation of a lateral ventricle when a colloid cyst, intraventricular hemorrhage, or postinflammatory adhesions block CSF flow. The close relationship between the foramen, the fornix columns, and the choroid plexus explains why endoscopic third ventriculostomy planning and transventricular approaches demand respect for memory-related limbic circuitry as well as venous anatomy.
